Requirements
for an Infant/Child Passport are:
For
the Passport Office(Required Documentation's are):
- Passport
Application Form DS-11
completed Online.
- Note: In the Mail Passport to Section,
fill in Arrow Travel & Tours address.
- Under Permanent Address Section, fill in
Applicants address.
- Do not sign this application until instructed
by acceptance Agent.

Submit
any of the below (Proof of Citizenship):
- Certified
U.S. birth certificate.
- Previous
fully valid U.S. Passport.
- Report
of Birth Abroad (Form FS-240).
- Certification
of Birth Abroad (Form FS1350).
- Certificate
of Citizenship or Naturalization from BCIS.
- A
Delayed Birth Certificate filed more than 1 year after your birth
may be acceptable if it:
- Listed
the documentation used to create it and
- Signed
by the attending physician or midwife or
- Lists
an affidavit signed by the parents or
- Show
early public records.
- Proof
of Identity (Present Evidence of Child's Relationship to Parents/Guardians
(NOTE: Previous US passports are not acceptable as proof of relationship
to the applying parent(s)/guardian(s). You'll need to submit one
of the following):

Certified
US birth certificate (with parents names).

Certified
Foreign Birth Certificate (with parents names & translation,
if necessary).

Report
of Birth Abroad (Form FS-240) (with parents names).

Certificate
of Birth Abroad (Form DS-1350) (with parents names).

Court
Order Establishing Custody.

Court
Order Establishing Guardianship.
NOTE:
If the parent(s)/guardian(s) name is/are other than that on these
documents, evidence of legal name change is required.
- Provide
Parental Identification:
- Valid
Drivers License (issued or renewed more than 6 month ago).
- Valid
Official U.S. Military ID.
- Valid
U.S. Gov't ID.
- Valid
U.S. or Foreign Passport with recognizable photo.
- Naturalization/Citizenship
Certificate from BCIS with recognizable photo.
- Alien
Resident Card from BCIS.
NOTE:
Your Social Security Card does NOT prove your identity.
- Present
Parental Application Permission Documentation:
- Both
parents (if the child is under 14) must appear together with
child at the Acceptance Agency and sign or
- One
parent appears, signs, and submit second parent's Statement
of Consent: Issuance of a Passport to a Minor Under Age 14,
Form DS-3053 authorizing
passport issuance for the child or
- One
parents appears, signs, and submits primary evidence of sole
authority to apply (such as one of the following):

Child's
certified U.S. or foreign birth certificate (with translation,
if necessary) listing only applying parent: or
- Consular Report of Birth Aboard (Form FS-240) or Certification
of Birth Abroad
- (Form DS-1350) listing only applying parent; or
- Court Order granting sole custody to the applying parent (unless
child's travel is restricted by that order); or
- Adoption decree (if applying parents is sole adopting parent);
or
- Court order specifically permitting applying parent's or guardians
travel with the child; or
- Judicial declaration of incompetence of non-applying parent;
or
- Death certificate of non-applying parent.
If
none of the above documentation is available, the applying parent/guardian
should submit Form DS-3053: Statement of Consent: Issuance of
a Passport to a Minor Under Age 14.
NOTE:
A third-party in LOCO PARENTIS applying on behalf
of a minor under the age of 14 must submit a notarized written
statement or affidavit from both parents or guardians authorizing
a third-party to apply for a passport. When the statement or affidavit
is from only one parent/guardian, the third-party must present
evidence of sole custody of the authorizing parent/guardian.
- Provide
two Passport Photos.
- Check/Money
Order of $120.00 payable to U.S. State Department & an Additional
$25.00 to the Acceptance Agency. Do Not write checks until instructed
to do so by Acceptance Agent.
- Provide
of International Departure; Original flight itinerary from the
airline or travel agent (Departure must be within 10 business
days for those requesting a passport in less than 2 weeks).
- Fill
out & Signed Arrow Travel & Tours Letter
of Authorization.

Before
going to Acceptance Agent, verify that you have collected the following
document:
- Passport
Application Form DS-11
completed Online.
- Proof
of Citizenship.
- Proof
of Identity.
- Two
Passport Photographs.
- Arrow
Travel's Authorization Letter for passport processing.
- Travel
Documentation (Travel Itinerary).
- Government
Fee ($120.00 for US Department of State & $25.00 for Acceptance
agency).
Once
you received the Sealed Envelope from the Acceptance Agency, please
call us before sending package with Payment to Arrow Travel &
Tours and a Pre-paid return FedEx Airbill or appropriate
fee.
Inside
the FedEx envelope there should be:
- Sealed
Envelope.
- Arrow
Travel & Tours order form.
- Payment
to Arrow Travel & Tours.
- Pre-paid
return FedEx Airbill or appropriate fee.
Note:
Sealed envelope expires 5 business days after being sealed by clerk
of court or postmaster.
