You
may need a New or First Passport if:
- You
have never had a passport before.
- Have
lost a valid passport.
- Passport
is currently expired & was issued more than 15 years ago.
- Passport
is currently expired & was issued prior to your 16th birthday
or you are
under age 16.
- Your
passport is damaged or mutilated.
- Your
name has changed since your passport was issued and you do
not have a
legal document formally changing your name.
Requirements
for a New or First Passport are
For
the Passport Office (Required Documentation's are):
- 2
(two) passport sized photos.
- Proof
of US Citizenship - Original/Certified Birth Certificate or
Naturalization
Certificate or a previous US Passport.
- Proof
of Identity - Valid Driver's License - issue more than 6 months
ago (not
temporary or learner's license) or Previous US
passport or Certificate of
Naturalization or government employee photo ID.

- Passport
Application Form DS-11
completed Online.
- Note: In the Mail Passport to
Section, fill in Arrow Travel & Tours address.
- Under Permanent Address Section, fill
in Applicants address.
- Do not sign this application until instructed
by acceptance Agent.
- For
those who have lost a valid passport, you need to also complete
Lost Passport
Form DS-64.
- For
those who have a mutilated or damage passport, you will need
to complete the
Statement of
Mutilation Form.
- Proof
of International Departure - Original flight itinerary from
the airline or travel
agent (Departure must be within 10 business days for
those requesting a passport
in less than 2 weeks).
- Check/Money
Order of $135.00 payable to U.S. State Department &
an Additional
$25.00 to the Acceptance Agency. Do Not write
checks until instructed to do so
by Acceptance Agent.
- Fill
out & sign Arrow Travel & Tours Letter
of Authorization.

Note:
Instructions
Take the above mentioned Required Documentation's of the Passport
Office to an authorized Acceptance Agent (Post Office, Clerk of
Court, Clerk of county), indicating that your passport application
will be hand-delivered to the passport office. Ask them to execute
the application and seal them in an envelope marked "TO BE
OPENED BY PASSPORT OFFICE PERSONNEL ONLY". Do not leave
sealed envelope with the clerk. You must present your original
proof of photo identity to Acceptance Agent.
All applicant 13 years or younger must appear in person at the
Acceptance Agent. Minor must be accompanied by both parent or
legal guardian with acceptable proof of photo ID. Send paperwork
plus sealed envelope to Arrow Travel & Tours for processing.
Once
you received the Sealed Envelope from the Acceptance Agency, please
call us before sending package with Payment to Arrow Travel &
Tours and a Pre-paid return FedEx Airbill or appropriate
fee.
Inside
the FedEx envelope there should be:

Envelope.

Arrow
Travel & Tours order form.

to Arrow Travel & Tours.

Pre-paid
return FedEx Airbill or appropriate fee.
Note:
Sealed envelope expires 5 business days after being sealed by
clerk of court or postmaster.
